Social Care Access delivers front line Adult Social Care services seamlessly in partnership with citizens and Hertfordshire County Council, from the first enquiry, through assessment and provision of service. Our focus is on what matters to the citizen, understanding their potential within their communities and striving to connect them to what matters most.  Our model incorporates a highly skilled, proficient workforce that delivers on its promises and provides innovative solutions to the social care sector.  


To ensure our success we are now recruiting for an Advanced Practitioner to join our team and continue to deliver on this incredibly important work.   From a technical / specialist perspective you'll contribute to and support the development of practice, procedures and policy and the professional development of your team.  You'll be accountable for the practice undertaken within the team, providing effective practice supervision in addition to performance management and appraisals. In addition, you'll be an SME and provide expert information, advice and guidance in complex situations and use sound knowledge of assessing, planning, commissioning and brokering services to meet needs and deliver positive outcomes.


Another key element of the role is to manage and monitor Key Performance Indicators and Measures relating to the contract and incorporate them into their Performance Management and oversight your team. 


This is an office based role with the opportunity to work from home on a hybrid basis.


What you need to do the job

We are looking for an experienced, qualified Occupational Therapist or Social Worker who is registered with the relevant association (Social Work England or HCPC/College of Occupational Therapists).   If you are also experienced in leading, managing and motivating a team, we want to hear from you!


There are clear progression and mentoring schemes in place which help provide opportunity to progress. We encourage you to take students and mentor unqualified staff; providing support and learning to others whilst developing your learning and experience. There are training opportunities to refresh and expand your knowledge. Some of the courses include single handed care training, safeguarding, moving and handling, mental capacity, plans and drawings for housing adaptation etc, as well as the opportunity to attend relevant forums and workshops.
