Role Purpose

An opportunity has arisen within the START team at Borders General Hospital.

You will work alongside Social Workers, para professionals and NHS staff, undertaking assessment, care management and practical interventions to safely discharge service users from hospital to home. You will provide appropriate advice, guidance and practical support to individual carers and adhere to legislation, standards and procedures and ensure effective use of resources.

Essential

Applicants must possess a recognised professional qualification in Social Work, Occupational Therapy, or Nursing

A current driving licence and use of a vehicle is an essential requirement of this post.
