Are you an experienced RMN or a preceptor with a passion for supporting people in later life and an interest in working in the community? The South Glos Community Mental Health Team has a vacancy for a part time (22.5 hours per week) Community Psychiatric Nurse.

We understand the importance of work life balance and offer some home working and the ability to work flexibly within a 3 day week.

We are an incredibly supportive multi-disciplinary team with many years of experience between us. We will provide you with monthly managerial and clinical supervision and access to training and development opportunities. As a team we try to support specialist interest in line with service demand, staff development and CPD.

Main duties of the job

 

We work with older adults with a severe functional or organic mental illness who live in their own homes, and work closely with our in-patient wards to support with discharge planning and facilitation. We also work closely with our Dementia Enhanced Support Team who provide support for people in crisis.

You will be focused on providing psychosocial and pharmacological interventions and treatments to our service users, their carers and families, following thorough person centred assessment and care planning, working within overarching principles of CPA. The focus of the team is to work within the Recovery model.

You will need to provide clinical leadership and supervision to Support Workers, and be able to work autonomously as care coordinator for a defined group of service users.

We also provide structured placements for student nurses from the University of the West of England and you will provide mentorship for learners.
