Are you looking for an exciting opportunity to join a large organisation working as part of a professional supportive team? if so Defence Primary Health Care (DPHC) Central and Wessex region are looking for a dynamic, organised and enthusiastic Band 5 Practice Nurse who will be working within our Primary Health Care team at Warminster. As the successful candidate, you will work as part of a multidisciplinary primary healthcare team (MDT) delivering primary healthcare nursing including elements of occupational health. As the Band 5 Practice nurse you will work with the Senior Practice Nurse or Senior Medical Officer within the medical facility (MF). Depending upon qualifications and experience you will be responsible for the delivery of appropriate assessment treatment, screening and advice to patients within their professional competence and in accordance with DPHC clinical guidelines and protocols. As the successful candidate your duties include but are not limited to: Providing a high standard of nursing care to all patients. Assessing, planning and implementing care for patients with minor health needs. Triage, treatment and referral if required to patients attending the medical centre. Once trained and assessed competent undertaking of preventative health programmes e.g. travel and vaccination clinics, new patients screening, audiometry. Supporting the care of patients with chronic illnesses e.g. Asthma, Diabetes, Coronary Heart Disease (CHD) and Hypertension ensuring continued maintenance of the Chronic Disease Register. Providing advice and supporting patients, their families and other healthcare professionals following assessment, diagnosis and through treatment. Developmental and training opportunities will be provided. The selection at sift stage will be based on your CV. The successful candidate will be required to provide evidence of the following: Professional/ clinical knowledge acquired through training to degree/diploma level. Registered Nurse with live registration on the NMC register. It will also be desirable to demonstrate experience in: Post Registration experience in Primary Healthcare. Family Planning. Travel Health. Immunisation and Venepuncture experience. Vaccination and Screening. Use of Medical Computer Systems.
