We have an opportunity for someone to join the Primary CareMental Health Team at HMP Eastwood Park which is made up of Non Medical Prescribers , a Primary CareMental Health Practitioner and a Social Prescriber. Your main responsibility will be completing triage assessments, supporting women in managing primary caremental health needs and working closely with other disciplines in a trauma informed environment.
This service will be based within HMP Eastwood Park. The Team were recently commended in a HMIP/ CQCvisit. Despite its country location, and tranquil atmosphere, Eastwood Park is in close proximity to the cities of Bath, Bristol, and Gloucester.
Main duties of the job
The Primary CareMental Health Team work alongside colleagues from Crisis Team, Therapies, Dual Diagnosis Practitioner, Admin, Neurodevelopmental Practitioner, Psychiatrists and Students. We also work very closely with non health colleagues and physical health professionals.
To provide supportive interventions to both individual and groups under the direction and supervision of the MH Team Manager and Band 7 NMP
To plan, organise and teach psycho-educational groupscovering a wide range of psychological knowledge and skills.
Always under the overarching framework of CPA, the post holder will be contribute to the on-going assessment, planning delivery and review of activities and interventions against identified health, social careand wellbeing needs, acting as care/ for a defined group of service users.
The role will also require undertaking and delivering specific health or social careassessments and interventions, according to care clusters, including identified service users on other caseloads. This maybe either on a one to one basis, or as part of a group activity.
