Community Connector (Social prescribing link worker)

£23,080 for 37 hours per week

Fixed term until March 31 2022 (contract extension to be confirmed)

Based in South Rural Suffolk locality

Do you enjoy developing communities to help reduce isolation and loneliness? Are you used to building relationships with professionals from health, social care and the voluntary sector? If so, come and join the ‘Connect for Health’ service and help benefit communities in the South Rural Suffolk (Shotley area). 

We are looking for a Community Connector with the experience, skills and confidence to engage people and build positive links within local communities.

As a Community Connector, you will work one-to-one with people in various settings, such as GP practices, on board Suffolk Family Carers’ mobile information centre or at their home (subject to Covid guidelines).

This is a great opportunity to get involved in projects that are making a difference to people living in Ipswich and East Suffolk. The Connect for Health Scheme is funded by Ipswich and East CCG and delivered in partnership with VCSE partners across Ipswich and East Suffolk. The programme supports people to improve their health and wellbeing, connect with their local community and find their own solutions. 

It is an exciting time for Social Prescribing in Suffolk with the developments of   the new ‘Connect for Health’ Ipswich Hospital pilot project and a dedicated service supporting individuals within BAME communities.
