As a Reablement Coordinator, you will be line managing team of Reablement Support Workers, using ICT daily, managing rota cover, completing daily quality audits, administering medication and when required assisting with personal care and meal/drink preparation.
Grays Fair Court is a care establishment consisting of 20 Accommodation Based Reablement Beds and 34 Housing with Care flats. Grays Fair Court Accommodation Based Reablement service provides person-centred, accommodation-based support for a period of up to six weeks to help people regain as much independence as possible and return home. Grays Fair Court offers 34 housing with care flats. All flats are self-contained with bedroom, lounge, kitchenette and bathroom. The Housing with Care tenants also have access to 24 hour assessed care.
Grays Fair Court is part of the wider Norfolk First Response Service which consists of Norfolk First Support, Swift Response, Supported Care and Benjamin Court (Accommodation Based Reablement Unit).
