Rapid Response Health Care Assistant

Job Description

We are looking for a keen and enthusiastic individual to join the Waltham Forest Rapid Response Team .

You will be part of a dynamic and flexible service providing unplanned and urgent care to patients in their own home

This is an excellent opportunity for a motivated and dedicated individual to join our well established team to support and assist in the provision of nursing care for patients in their own home. The team support patients through acute episodes of ill health as an alternative to hospital, providing enhanced and complex care in a crisis, and handing stabilised patients to planned care colleagues e.g. GP, DN, community matron, physio etc.

The successful person will be required to visit people in their home to provide care and deliver recovery treatment and pathways for hospital admission avoidance.

For the successful candidate there will be opportunity to gain new skills and develop their practice in a supportive environment as a valued member of the team.

You must be flexible, a good team worker, used to working under pressure, have excellent interpersonal skills and be able to communicate effectively with patients, carers and other members of the MDT.

Rapid Response is a 24/7 service including weekend and bank holidays. Team members will be expected to work shifts on a set rolling rota basis.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

 

Assessment/Interview info

At NELFT we set very high standards to ensure we provide quality services. We require prospective employees to carry out an online literacy and numeracy test, equivalent to NVQ Level 2. Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction.

 

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £3,722 to a maximum of £4,743).
