Senior Social Prescriber

(Known internally as Senior Community Connector)

£25,991 pro rata per annum (£17,561 per annum based on 25 hours per week)

25 hours per week usually covering Monday to Friday

Initially 6 month contract from start date

Based at James Paget University Hospital

 

Suffolk Family Carers is an innovative and creative charity, recognised nationally as the overall GSK Impact Award winner 2019.  We work in partnership with over 100 organisations to deliver a range of services that impact positively on the lives of adult, young adult and young carers.

 

It is an exciting time for Social Prescribing in Suffolk with a development of a new ‘Social Prescribing’ pilot project.  Suffolk Family Carers is building on our successful Social Prescribing programme in Suffolk and is looking to develop and deliver this service within James Paget University Hospital.  This post will link very closely to our existing Social Prescribing teams within Suffolk Family Carers and across all partnership areas.

 

The Social Prescribing programme supports people to improve their health and wellbeing, connect with their local community and find their own solutions.

 

Are you passionate about helping people to identify what matters to them?  Are you looking for a new and exciting challenge, to utilise your health coaching/coaching skills? Do you enjoy the challenge of being involved in the implementation of a new project to help patient back into their community, prevent readmissions to hospital and reduce isolation and loneliness?

 

As a Senior Community Connector, you will work one-to-one with people within the hospital (and possibly in the community) to aid a smooth timely discharge. We are looking for a Senior Community Connector with the experience, skills and confidence to engage people and build positive links within the hospital and local communities
