About us

Modality Partnership is a national GP super partnership serving more than 450,000 patients across 9 regions of the UK.  In Primary Care Network (PCN) Terms, we are a super PCN covering circa 1 million patients across the country.  Embracing the NHS ethos Modality Partnership is recognised nationally as an innovator and leader in transforming primary care.  We work in partnership with non-Modality practices, Acute Trusts, and other Stakeholders.   Our vision is to be the leader in delivering resilient community-based services to improve population health across the system.  We are a group of likeminded professionals working together to make a difference.  Our shared values of CARE, Commitment, Accountability, Respect and Excellence ensure that we all work as one with compassion, honesty, valuing differences, and fairness providing excellence through hard work and dedication.
