Job Description

The Growth Company (GC) has a new vacancy for a Key Worker to support individuals to make sustained changes to their lifestyles which, in turn, improve their health & wellbeing.

We help to make a lasting difference to the lives of people throughout the North of England and beyond, by supporting individuals to gain meaningful employment.

Our highly skilled advisers and keyworkers work with a range of individuals; from people who have found themselves recently out of work, to those with complex barriers such as physical disabilities, mental ill-health and debt.

Key Responsibilities 

  • Ensure service users receive a personalised Wellbeing offer that meets individual needs/levels of complexity
  • Manage a wide range of service users, by caseload, to achieve targets
  • Deliver Comprehensive Assessments, including screening for LDD
  • Identify, develop and maintain effective relationships with key local services to support service users, including signposting, case conferencing
  • Deliver support to service users via appropriate channels, including digital, group workshops and face-to-face sessions
  • Produce high-quality service user action plans
  • Plan, deliver and evaluate group sessions for service users
  • Ensure Wellbeing contract compliance
  • Take part in observations, assess own delivery, and act on feedback, including undertaking CPD 

About You 

  • Developing strength-based action plans Detailed knowledge of the local health and wellbeing support landscape (statutory and community)
  • Ability to work in a target-driven environment, with a focus on the successful achievement of outcomes
  • Ability to use digital channels and platforms in service delivery
  • Strong interpersonal skills - service user contact, team and partnership working

Skills Required:

  • Hold Substance Misuse Awareness Level 3
  • Recent experience of delivering personal wellbeing progressions and outcomes, ideally in a Criminal Justice setting
  • Experience of managing a caseload of service users, to support them with improving their wellbeing
  • Counselling Skills for Health Professionals
  • Suicide Awareness/Prevention
  • Working with victims and perpetrators of Domestic Abuse
  • Use of ITEP/Node-Link mapping as behaviour change tool
